Health and Safety

In the Upper Grand DSB, we use the IRS - the Internal Responsibility System - which means that all health and safety concerns should first be reported to your supervisor (Principal, in most cases). If sufficient action is not taken to remedy the health and/or safety concern, please contact your school's Health and Safety representative (see your Health and Safety board for your school rep). If you still need support, please contact the TBU Health and Safety Officer or the D18 Health and Safety Officer (contact info on the board).

 

It is recommended that you report in writing; while email will work for many situations, UGDSB has a formal reporting system for accidents and incidents. Members need to file these reports in order to appropriately document accidents and incidents (in some cases, if it has not been properly documented, then members may not be able to access appropriate supports). Please review the following list to help you determine which form to complete (note that you will need to log into the UGCloud/Sharepoint to access the forms). 

Section 1 of the Occupational Health Safety Act (OHSA) defines "Workplace Violence" as any of the following:

  • the exercise of physical force by a person against a worker, in a workplace, that causes or could cause physical injury to the worker;

OR

  • an attempt to exercise physical force against a worker, in a workplace, that could cause physical injury to the worker;

OR

  • a statement or behaviour that it is reasonable for a worker to interpret as a threat to exercise physical force against the worker, in a workplace, that could cause physical injury to the worker.

Members who experience a scenario that meets any of these definitions of workplace violence are encouraged to fill out the appropriate form as described below.

 Was this an ACCIDENT? 

      Did you have an accident while at work? 

Complete the EMPLOYEE INCIDENT REPORT if you had an accident (slip/trip/fall/cut/etc.) at work. You may or may not have required first aid or any other medical attention at the time, but it is in your best interest to report all accidents in case of complications later.

      Did you witness another Member's accident?  

Complete the ACCIDENT/INCIDENT WITNESS REPORT if you witnessed a Member’s accident.

 Was it a serious INCIDENT involving a STUDENT? 

        Did you have a physical incident or experience a threat of a physical            incident with a student?

Complete the EMPLOYEE INCIDENT REPORT (formerly called the Violence Incident Reportfor incidents where a student uses physical force, attempts physical force, or makes a verbal, written or electronic statement that could be reasonably interpreted as a threat of physical force including aggressive action.

         AND

Complete the SAFE SCHOOLS INCIDENT REPORT

        Did you have a non-physical incident with a student?

Complete the SAFE SCHOOLS INCIDENT REPORT if the incident you reported on above might lead to a suspension or an expulsion.

        Did you witness a physical incident involving a student(s)?

Complete the SAFE SCHOOLS INCIDENT REPORT if the incident you reported on above might lead to a suspension or an expulsion.

        Did you use physical force to control a student’s aggressive behaviour?    

Complete the PHYSICAL INTERVENTION REPORT if physical intervention was used to control a student’s aggressive behaviour.

        Did you witness another Member's incident with a student?             

Complete the ACCIDENT/INCIDENT WITNESS REPORT if you witnessed a Member’s serious incident with a student.

 Was it an INCIDENT with a NON-STUDENT?   

        Did you have an incident with a non-student?                      

Complete the WORKPLACE VIOLENT INCIDENT REPORT (417-1) Non-Student for incidents where a non-student (visitor, volunteer, parent, etc.) uses physical force, attempts physical force, or makes a verbal, written or electronic statement that could be reasonably interpreted as a threat of physical force including aggressive action.

        Did you have an incident with a non-student?                      

Complete the ACCIDENT/INCIDENT WITNESS REPORT if you witnessed a Member’s incident with a non-student.
